Resident Care

At Snyder Village Health Center, the comfort and care of residents is our top priority. Our exceptional nursing staff provides 24-hour coverage, works hand-in-hand with area physicians, and is knowledgeable in all aspects of skilled care. Our Certified Nursing Assistants are trained to take care of the specific needs of each resident, whether they require short-term or long-term care.r residents.

Rehabilitation Care/Transition from Hospital Stay

Our Admission Liaison works closely with area hospitals to transition patients in need of rehabilitation services at our Health Center. We hone in on the specific needs of each resident to follow his or her care plan. Health Center residents may benefit from the variety of therapy services offered on-site, including physical therapy, occupational therapy, and speech therapy.

Memory Care

Memory Care in our Health Center is a secure wing designed especially for residents who are diagnosed with dementia, Alzheimer’s Disease, or other severe memory-related issues. The features and structures of this hall ensure not only care and security but provides the interaction and support to ensure the highest quality of life for our residents. Our goal is to preserve our residents’ independence and dignity while engaging them socially with enjoyable and stimulating activities.

Diagnostics and Wound Care

In addition to providing for the daily needs of our residents, Snyder Village also offers diagnostic services, such as EKG’s, x-rays, and lab work. Our professional, experienced staff can also administer IV therapy and care for tracheostomies and feeding tubes. We offer specialized wound care for residents who may have fragile skin or who are dealing with an injury, disease or incision. Our Wound Care Nurse ensures proper treatment and preventative measures as well as monitoring.

Hospice Care

Hospice is a type of care for those with a serious illness or who are approaching the end of life. Hospice care focuses on quality of life as well as the comfort and dignity throughout the dying process. Hospice care generally focuses on helping patients live each day as good as it can be. Snyder Village partners with various hospice agencies who assist residents and families with making every moment count with their loved ones.

Respite Care

Snyder Village Health Center provides short-term care for loved ones when their regular caregivers are not available. Respite Care can fill the gap during vacations, business trips or for any reason that your loved one may need care.
