- Limited medical care (level of care based on assessed need)

- 24-hour skilled nursing care
- Highest level of care available
- Assistance provided for residents who may be bedridden, wheelchair-bound, or need daily medical attention
- Shared or private rooms available
- All-inclusive (meals, housekeeping, & maintenance included)
- Full social calendar with activities & entertainment
- Medicare, Medicaid, or private pay accepted
Memory
Care
- Specialized care for those with cognitive impairments, such as Alzheimer's or other forms of dementia
- Two Memory Care programs offered (level of care based on assessed need)

- 24-hour nursing care
- Increased emphasis on security & safety precautions

Care
- Care for individuals wishing to stay in own home but in need of non-medical help
- Help with personal care (bathing, dressing, or other activities of daily living), transportation, companionship, and/or housekeeping
- Hourly rate charged
- Available in Peoria, Tazewell, Woodford, & Marshall Counties and on the Snyder Village campus
- No medical care provided
Therapy
- Physical therapy, speech therapy, & occupational therapy offered
- Outpatient services available to the community & Snyder Village residents
- Rehab after a hospital stay/Inpatient care available
- Available in Peoria, Tazewell, Woodford, & Marshall Counties and on the Snyder Village campus
- Medicare & most insurance providers accepted
